In my surveys i used a description text as in your suggestion. I only thought it would be semantically correct to put the question text into the question itself. Otherwise you will get an empty question in your survey report as result.
BTW: I like your survey page allot. Much better than any other free in the web.

You can add question above you rating question, and choose descriptive text. But you might have a point with your suggestion. Maybe there should be a leading descriptive text for rating questions, by default.
